This notice covers the website. Clinical records at the clinic are governed by PHIPA and the College standards of each profession.

Appointment requests and the after-hours patient form collect only what the front desk needs to call you back: name, phone, email, and a short reason for visiting. Do not send OHIP numbers, diagnoses, or other detailed health information through this website.

Requests are emailed to mississaugawellness@gmail.com and to the WordPress administration email using this site’s hosting mailer. Newsletter signups are used only for clinic updates.

We do not sell personal information. To correct or remove a website inquiry, email mississaugawellness@gmail.com.
